A client I encountered in my clinical setting experienced depression and was diagnosed with Major Depressive Disorder. The client had a history of multiple traumas, including childhood abuse and a recent loss of a loved one. The client was prescribed a combination of medications including fluoxetine (Prozac), quetiapine (Seroquel), and lorazepam (Ativan).
A problem that was not resolved with the treatment regimen was the client’s lack of motivation and energy. Despite being on multiple medications, the client was still experiencing severe feelings of hopelessness and had little to no interest in participating in therapy or engaging in activities. The reasons for this may include a lack of proper medication management, a lack of therapy tailored to the client’s specific needs, and the potential for comorbid conditions such as PTSD.
An effective nursing intervention that I observed was the use of motivational interviewing. The nurse used this technique to explore the client’s ambivalence and resistance to change, and helped the client identify their own reasons for wanting to improve their mental health. This intervention was effective because it helped the client to identify their own goals and desires, which motivated them to take more active steps towards recovery.
I do feel that this client was kept safe while under care. The client was closely monitored for any suicidal ideations and was placed on a safety plan as needed. The client was also provided with appropriate referrals for therapy and other resources to assist with their recovery.
