How would you apply the principles of patient-centered care to the provision of nursing health promotion services in the rural community?
• Describe a patient-centered care approach to the provision of nursing health promotion services in relation to this specific rural health issue.
• Provide an example of primary, secondary, and tertiary nursing services to improve outcomes in this specific issue.
The principles of the patient-centered care model can be applied in the overall provision of health care services within the rural communities through various ways. Increased access to health care services is a levant and would help the rural patients to get quality nursing services. Creation of awareness and education of the rural patients is important. Patients are able to get information and knowledge on the various nursing programs that would be able to improve the health. The involvement of family and friends is also recommendable (Elwyn, et al., 2014). It involves the expansion of the social support programs that helps in reducing stress often faced by the patients in the rural communities.
Moreover, the integration and coordination of care services would help in improving nursing services. For example, focus groups can be used to improve the effective coordination of health care to improve health care services. Coordination of care services would be able to improve the various care services in rural community including the clinical care, support services, and the front-line patient care services. These methods are related to the principles of the patient-centered care approach to expand the delivery of nursing care services in rural community.
The challenge of accessing emergency care services is a particular rural health concern that can be addressed through the implementation of the patient-centered care model to deliver quality nursing health promotion programs. The patient-centered care approach will involve the creation of mobile clinic across the rural community to help in providing emergency care services (Liberati, et al., 2015). The mobile clinic services are effective in improving the efficiency and effectiveness of nursing health promotion services.
An example of primary, secondary, and tertiary nursing services that would improve emergency services includes the provision of quality emergency care services. An example of primary services is the local stakeholder partnerships and collaboration should be part of the process of offering emergency care services (Constand, MacDermid, Dal Bello-Haas, & Law, 2014). It would improve the capacity of the health clinics to deliver quality care to the rural community. The secondary care services are the emergency care services that helps in improving care services. Tertiary nursing services that should be advanced is the surgical care services to help inn improving the nursing care services.
