FlexPath Assessment Brief

Course: NURS-FPX 6021: Biopsychosocial Concepts for Advanced Nursing Practice 1
Assessment Number: 3
Assessment Title: Intervention Development and ProFessional Advocacy Narrative
Due Date: [End of Unit 8]

Assignment Instructions

Following your in-depth analysis of a health inequality, this assessment requires you to translate that knowledge into actionable tools and strategic communication. You will develop a detailed intervention guide based on your proposed initiative from Assessment 2 and create a narrative script to advocate for its implementation to a key stakeholder audience.

Scenario

Your proposal from Assessment 2 has garnered initial interest from the QI/Ethics Committee. You have been asked to develop a concrete implementation guide for the clinical staff and to present a succinct, powerful overview to the hospital’s executive leadership team to secure final approval and funding.

Instructions

Develop two distinct deliverables as part of this assessment:

Deliverable 1: Evidence-Based Intervention Implementation Guide
Create a 3-4 page guide intended for the nurses and other healthcare staff who will be implementing your proposed initiative. The guide must be practical, clear, and grounded in evidence. It should include the following sections:

  1. Initiative Overview & Objectives: Briefly restate the health inequality, target population, and the primary and secondary objectives of your initiative (using SMART criteria: Specific, Measurable, Achievable, Relevant, Time-bound).

  2. Step-by-Step Implementation Protocol: Outline the specific steps for executing the initiative. This should be a practical, chronological guide. For example:

    • Patient Identification and Screening Process

    • Initial Assessment and Engagement Strategy

    • Key Interventions or Actions to be Performed

    • Frequency and Duration of Follow-up

    • Documentation Requirements and EMR Integration Tips

  3. Interdisciplinary Collaboration Plan: Specify the roles and responsibilities of at least three different interdisciplinary team members (e.g., Advanced Practice Nurse, Social Worker, Community Health Worker, Primary Care Physician, Dietitian). Explain how they will coordinate care.

  4. Evaluation Plan: Define how the success of the initiative will be measured. Identify 2-3 key metrics (e.g., HbA1c reduction rates, hospital readmission rates, patient satisfaction scores) and describe the method and timeline for data collection and review.

Deliverable 2: ProFessional Advocacy Narrative
Write a 2-page narrative script for a 5-7 minute presentation to the executive leadership team (e.g., CFO, CNO, CEO). The narrative should be persuasive and focused on the “why” from a systems perspective. It must include:

  1. A Compelling Hook: Begin with a powerful statistic or a brief, anonymized patient story that highlights the human and financial cost of the current inequality.

  2. The Proposed Solution: Clearly and concisely summarize your initiative, emphasizing its innovation and alignment with the institution’s mission and financial goals (e.g., value-based care, reducing penalty readmissions).

  3. The Ask and Justification: Explicitly state what you are asking for (e.g., funding, staff time, IT support) and justify it with a brief cost-benefit or return-on-investment argument, citing your evidence.

  4. Conclusion and Call to Action: End with a strong, forward-looking statement that urges the leadership to approve the initiative, reinforcing the positive impact on patient outcomes and the organization’s reputation.

Formatting Note: The entire submission (Guide + Narrative) should be 5-7 pages total. Adhere to APA 7th edition formatting for the entire document, with separate title pages not required for each deliverable within the single document. A minimum of four new peer-reviewed sources, published within the last five years, is required, in addition to any relevant sources carried forward from Assessment 2.
