Assignment Description

Assessment Goal: Develop a preliminary, patient-centered care coordination plan for a selected community health concern, identifying related physical, psychosocial, and cultural factors and available community resources.

Instructions:

1. Select a Health Concern: Choose a specific, prevalent health concern impacting a community population (e.g., Type 2 Diabetes Management, Opioid Use Disorder, Childhood Obesity, Hypertension).
2. Analyze the Problem:
   • Describe the Health Concern: Provide an overview of the selected health issue, detailing its impact on individuals and the community.
   • Identify Factors: Analyze and articulate the physical, psychosocial, and cultural considerations that influence the management and outcome of this health concern within the targeted population.
3. Develop the Preliminary Plan:
   • Establish Goals: Propose at least two to three specific, measurable, achievable, relevant, and time-bound (SMART) goals for the patient/population related to the selected health concern.
   • Identify Resources: Research and list three to five real, available community resources (e.g., support groups, free clinics, government programs, specific non-profit organizations) that can contribute to a safe and effective continuum of care for this health concern. Explain how each resource supports the goals.
   • Propose Interventions: Briefly suggest preliminary, evidence-based nursing interventions that would use Health Information Technology (HIT) to support the care coordination goals and address the identified factors.
