To create a Patient Teaching Plan aimed at educating a specific patient population about a specific health topic. This plan will be used to develop a Visual Teaching Tool in a future assignment , recognize the influence that developmental stages have on physical, psycho social, cultural, and spiritual functioning. Health Topic: Medication Safety at Home
Importance: Medication safety at home is critical to prevent adverse drug events, medication errors, and improve health outcomes. Evidence shows that many medication errors originate during administration in the home (Author, year). Educating patients on proper medication administration, storage, disposal, side effects monitoring, and more can improve compliance and reduce risks.
Population: Adults over age 65 living independently at home with multiple chronic conditions requiring complex medication regimens (more than 5 medications daily).
Barriers: Physical barriers like impaired vision or hearing, reduced mobility, or inability to open containers. Educational barriers like poor health literacy or cognitively impairment. Lack of social support at home. Difficulty affording medications.
To address barriers, teaching will be done verbally one-on-one using large print education sheets. Caregivers will be included. Tools like medication organizers, magnifiers, and audible alarms will be recommended. Financial assistance resources will be provided.
Setting: Education will occur one-on-one during an office visit at an outpatient primary care clinic.
Learning Objectives:
Correctly sort medications by time of day or purpose following education.
Identify and explain three high risk situations for medication errors in the home setting.
Demonstrate proper medication list updating and reconciliation practices at provider visits.
Evaluation: Patients will fill daily medication organizers correctly to assess learning objective #1. They will complete a written multiple choice quiz to assess objectives #2 and #3 at the end of the education. Effectiveness can also be evaluated through patient reports of errors or adverse events at follow up visits after starting new high risk medications.
