In this assessment, you will define the patient, family, or population health problem that will be the focus of your capstone project. You will evaluate the problem from the perspectives of leadership, collaboration, communication, change management, and policy.

Preparation:
For this assessment, you will assess the patient, family, or population health problem selected for your capstone project. Plan to spend approximately 2 hours working with your chosen patient, family, or group to explore the problem in terms of leadership, collaboration, communication, change management, and policy. During this time, you may also choose to consult with subject matter and industry experts to gain insights into the problem.
