CONSTITUTIONAL/GENERAL APPEARANCE: Vital signs stable, in no acute distress. Alert, well developed, well nourished.
HEENT:

COVID:
As of today, over 400 million confirmed cases of COVID-19 globally since the start of the pandemic.
In the US, over 78 million confirmed cases and over 950,000 deaths.
The dominant variant currently circulating globally is Omicron BA.5, which is more transmissible than previous variants. However, it also appears to cause less severe disease in most people, especially those who are vaccinated and boosted.
Vaccination remains the best protection against severe illness and death from COVID-19. The new bivalent boosters which target the original virus and Omicron variants are now recommended for those 5+ months past their last vaccine or booster.
Strep throat:
Strep throat is caused by group A streptococcus bacteria and is usually treated with antibiotics like penicillin or amoxicillin.
Symptoms include sore throat, fever, headache, and stomach pain. The throat may appear red and swollen with white or yellow spots.
A rapid strep test can diagnose it within minutes. Left untreated, it can sometimes lead to rheumatic fever or kidney disease.
Influenza (flu):
Flu season in the northern hemisphere typically runs from October through May. Symptoms are similar to strep throat but also include body aches, extreme tiredness, and respiratory symptoms like cough and stuffy nose.
The flu is caused by influenza viruses, which mutate each year. Vaccination is recommended annually to protect against the strains predicted to circulate most that season.
Antiviral medications can treat the flu but work best when started within 48 hours of symptoms beginning.
Based on the symptoms described, COVID-19 and strep throat seem most likely differential diagnoses. Testing would be needed to confirm, given the similarities to flu as well.
S: CC: “My muscles ache and my throat hurts alot. Ive also had a fever of 100.6 since yesterday”
HPI: 13-year-old male presents with 2 days of sore throat, muscle aches, and fever of 100.6F. No other associated symptoms. No recent sick contacts. Up-to-date on vaccines.
O: Vitals: T 101.0F, HR 90, RR 18, BP 110/70. General appearance is uncomfortable but well-nourished. Oral exam reveals erythematous throat with exudates. Lungs CTA, heart RRR, abdomen soft NTND. No lymphadenopathy.
A: Sore throat. Differential diagnoses include strep throat, COVID-19, and influenza based on symptoms and season.
P: Throat culture ordered to confirm diagnosis. Educate on signs of worsening infection. Prescribe amoxicillin 500mg PO BID for 10 days. Advise Tylenol for fever and discomfort as needed. Follow-up in 5-7 days to assess response to treatment. Consider COVID-19 testing depending on local guidelines and risk factors.
